REQUEST FORMS

  • External Space Tabling Request Form

    Student Organizations/Clubs requesting for tabling outside of the U-SU (i.e. the bookstore, library, main walkway, etc.) should submit this form through Information & Event Services. These requests are faxed to the campus Facilities Use Coordinator for scheduling and confirmation. Spaces are available for Student Organizations/Clubs to provide the campus community with information regarding their Club/Student Organization, its activities and issues or concern. Off campus sponsor must contact the campus Facility Use Coordinator directly to request for a space for tabling.

    • Tables must be reserved through the Information & Event Services at least ten (10) days in advance.
    • Sponsors are prohibited from approaching or attempting to stop individuals on the walkway. Representatives must distribute information from behind their reserved table.
    • Sponsors are prohibited from soliciting donations.
    • Food and beverages may not be dispensed without prior approval from the Information & Event Services, U.A.S. U.A.S. Golden Eagle Hospitality Services, and the Environmental Health and Safety Department. Required forms are available at the Information & Event Services Office.
    • Unlimited reservations for External Tabling Information can be made.
    • Sponsors must provide their own equipment. The U-SU does not provide equipment to locations outside of the U-SU Plaza and U-SU Walkway
    • U-SU DOES NOT provide equipment to locations outside of the U-SU Plaza and U-SU Walkway.

    Information Tabling Request Form

    The Union Plaza Tabling and Walkway spaces are available exclusively for Student Organizations/ Clubs to provide the campus community with information regarding their Club/Student Organization, its activities and issues or concern.

    • If AMP Sound is desired, an Outdoor Event Request Form will need to be attached to this form.
    • If Tabling is not exclusively to provide information, this form is not applicable. Reservation would turn into an event and an Event Registration Form would be needed along with an Outdoor Event Form.
      *If food is served, a Temporary Food Permit Form will also be required.*

    On-campus and off-campus sponsors are not permitted to reserve tabling spaces in the Union Plaza. Those interested in Walkway Information Tables should contact the campus Facilities Use Coordinator for designated areas on campus where tabling is permitted.

    • The standard set up for U-SU tabling spaces are one (1) 6x18 foot table, one (1) canopy and two (2) chairs. *tabling in the U-SU Plaza does not include a canopy*
    • NO sponsor is allowed to bring personal equipment (EX: personal customized canopy).

    Meeting Room Request Form


    You need this form if you want to host a one-time general meeting in one of our meeting rooms at the University-Student Union. Please note you can only use one of our meeting rooms and event must be two hours maximum. General meeting reservations should not be submitted more than 15 business days prior to meeting date. The following are considered Events:

    • Event exceeds the 2 hour limit
    • Anything other than the club/org’s general meetings
    • Using 1 or more rooms
    • Having any type of food
    • Having special speakers
    • Using the U-SU Theatre

    You will need to submit an Event Registration Form along with a Meeting Room Request Form and a Temporary Food Permit (if you will be bringing food from off campus) *Food from UAS or bought on campus will not need a Food Permit, but a receipt will be needed.* All reservation inquiries require a completed Meeting Room Request Form and/or Student Organization Event Registration Form. Space will not be placed on hold or entered in the operating system without proper forms having been submitted first. Incomplete forms will not be processed. Reservation requests can be made in person, via fax (for on-can be made in person, via fax (for on and off-campus sponsors only), or online.

    Information such as the name of the sponsoring club, organization, department, or individual is required to determine group classification and applicable meeting space rental rates. Only authorized individuals that are specified on department contact lists may make reservations. Names not registered on our system will require the Department’s Chair to email the I&ES Coordinator with an updated list.This authorization includes the approval of fees and charges associated with any reservation. Please note that applicable fees may apply and are dependent on the scope of any event.

    Available hours for meeting room reservations are 7:30am – 9:30pm (weekdays), 7:30am – 6:30pm ( Saturdays), Closed (Sundays)

    To allow for proper scheduling and staffing, event details must be finalized ten (10) business days prior to the event. This includes any special arrangements such as complex audio/visual requirements, telephone or data lines, production technicians, lighting, etc. Changes in the physical set-up of reserved space must be submitted to the Information & Event Services Office no later than forty-eight (48) hours prior to the event. Labor charges will be incurred (depending on the extent of the change) if the changes are requested less than forty-eight (48) hours prior to the event.     Outdoor Space Request Form

    The U-SU Plaza Stage is a designated public forum area. A “public forum” refers to University property that has traditionally been available to assembly or debate. Right of access and right of equality of access are guaranteed in these public forums. Prior restraint and most content-based restrictions are prohibited, but reasonable time, place, and manner regulations may be applied. Spontaneous events and demonstrations may take place in the public forum areas without reservations. Planned events and demonstrations in the public forum areas require reservations.

    Student Organizations and other on-campus entities may reserve the Union Plaza Stage, Union Plaza Spaces and Union Walkway Area for programming through the Information & Event Services Office. Equipment (tables, chairs, AV) can be arranged at the time the reservation is made and no later than two (2) days prior to the event. Equipment rental fees may be in effect if applicable. *Reservations for the Union Plaza Stage are not available during Finals Week or when there are concurrent or conflicting events on campus.*

    • Amplified sound is only allowed for a maximum of 2 hours per day.
    • Special permission for outdoor amplified sound must be obtained from the Operations Director or designee.
    • Sound will be monitored and cannot exceed 80 decibels.
    • Outdoor amplification is not allowed during Finals Week or when there are concurrent or conflicting events on the campus.

    Weekly/Bi-Weekly Meeting Request Form

    Student Organizations requesting reservations for weekly or bi-weekly meetings may submit their request one month before Organizational Development Conference (ODC) for Fall and Spring semester. To be considered a weekly meeting ensure that:

    • Meeting is held on the same day, same time, and same location.
    • Must be held in one meeting space only.
    • Cannot exceed two (2) hours in duration.
    • Minimum of three (3) meetings.
    • Meeting is held on the same day, same time, and same location.
    • *Double Series must be submitted in 2 individual forms*
    • *You may have food in your weekly or bi-weekly meetings as long as your meetings are in the U-SU and you submit your Event Registration Form and a Temporary Food Permit along with this form*
    University classrooms can be requested for use by student clubs and organizations for weekly/ bi-weekly meetings only. The request(s) should be submitted to the Information & Event Services Office, then it is sent over to the campus scheduling office for availability and confirmation. Please allow up to 10 business days for classroom requests.
